Responsible for assisting residents with all social and emotional issues relating to placement in a nursing facility and all other aspects of his/her life.
Performs periodic assessments of all residents to determine status and needs.

Qualifies as a Social Worker under the specified requirements of the State of Connecticut Public Health Code with a minimum of a B.
S.
in Social Work.
2.
Ability to read, write and comprehend English; ability to follow oral and written instructions.
3.
Interest in the psychosocial needs of the aged and the chronically ill, with the ability to work with the elderly.
4.
Ability to work with supervisors, co-workers and facility staff in the performance of duties.
5.
Ability to observe proper safety and sanitary techniques.
6.
Ability to work hours as scheduled based on requirements of the position/assignment.
7.
Excellent assessment, communication and counseling skills.
8.
Experience in a long-term care setting.
9.
Knowledge of state and federal regulations governing the social service function within a nursing facility.
WORKING CONDITIONS:
Works in an appropriately lighted and ventilated environment.
This position may result in your being exposed to blood/body fluids.
PHYSICAL REQUIREMENTS:
1.
Must be able to move intermittently throughout the day, proper body mechanics required -- bending, stooping, turning, stretching, and reaching above the shoulders are involved.
PRIMARY FUNCTIONS:
1.
Supervises the Social Work Department personnel as well as carries a case load of residents.
OTHER DUTIES:
1.
Participates in resident care conferences to assist in the implementation of care plans.
2.
Assesses all new residents to obtain history, establish specific goals and to complete the appropriate section of the Minimum Data Set (MDS).
Writes progress notes according to established schedules.
3.
Assists residents and families with adjustments which follow initial placement.
Encourages resident participation in facility activities, and participates in Family Council meetings for resident's benefit.
4.
Identifies and documents changes in affect, behavior or personality.
Advises the appropriate personnel.
5.
Assists in preparing residents and families with the implementation of the discharge planning process from the facility (when possible) and completes discharge home follow-ups according to established schedules.
6.
Intervenes in all resident conflict situations as necessary.
7.
Assumes the duties of the Director of Admissions in his/her absence.
8.
Assumes duties and attends meetings as directed by the Administrator.
9.
Assists with the management of the residents' finances and with securing funding for skilled nursing care services when indicated.

Coordinates and completes the MDS with appropriate CAAs and trigger worksheets; determines assessment reference dates.
2.
Reviews resident's medical history.
3.
Interviews appropriate unit staff.
4.
Reviews resident care plans using information available and resident care plan library during MDS completion window.
5.
Daily review of rehab software for unscheduled assessments What You Will Need:

Has working knowledge of MDS and PPS process 7.
Knowledge of Submission Process and Interpretation of Validation Reports 8.
MDS certified preferred.
